I have received a letter from the IRS along with a W-9 form. What do I need to do with this?

The IRS is advising that there is a discrepancy in the TIN reporting. An individual's TIN is his or her social security number. The IRS considers a TIN as incorrect if either the name or number shown on an account does not match a name and number combination in their files or the files of the Social Security Administration.

Compare the name and social security number on your account with us with the name and social security number shown on your social security card. Then use the chart that came with the Notice to decide what action to take. Once the W-9 is completed, simply return the form to the Delta Community Credit Union.

If you have further questions regarding this notice and the W-9 form you can call the IRS for more information at 1-800-829-8374.
